Download the forms below for the test(s) you are registering for, all forms are located on our website with the exception of the Eye Forms.  If you are having Eye Exams done, you will be sent a paper form prior to have filled out for that day.  FORMS WILL NOT BE PROVIDED DAY OF.

Contact us at healthclinic@sylvanvet.biz to register for the May Clinic, please include the following information in your email:

• Owner Information: Name, Address, Phone Number, Email
• Dog Information: Call Name, Breed, DOB, Sex
• List of tests/procedures being performed on each individual dog, if more than one 

You will receive an email verification of receipt.  Once the clinic registration closes you will receive an email directly from healthclinic@sylvanvet.biz detailing your appointment time and the amount you will owe that day. Please disregard any automated emails you receive from our clinic, these may show a different time than what you are actually scheduled for. Credit Cards will be the only accepted form of payment.

CLINIC REGISTRATION CLOSES MONDAY APRIL 3rd

If you need you make any changes to the services being performed please do so no later than APRIL 17th or we may not be able to accommodate your additional requests.
