Download the forms below for the test(s) you are registering for, all forms are located on our website with the exception of the Eye Forms.  If you are having Eye Exams done, you will be sent a paper form prior to have filled out for that day.  FORMS WILL NOT BE PROVIDED DAY OF. 

Contact us at healthclinic@sylvanvet.biz to register for the March Clinic, please include the following information in your email:

• Owner Information: Name, Address, Phone Number, Email
• Dog Information: Call Name, Breed, DOB, Sex, please indicate if your dog is over 90lbs
• List of tests/procedures being performed on each individual dog, if more than one *BE SPECIFIC- SIMPLY STATING OFA'S DOES NOT AUTOMATICALLY MEAN YOU ARE HAVING FILMS DONE

You will receive an email verification of receipt.  Once the clinic registration closes you will receive an email directly from healthclinic@sylvanvet.biz or ashley@sylvanvet.biz detailing your appointment time and any other pertinent details you may need.  You will also receive a digital appointment and anesthesia release form to your email so be on the lookout for those. PLEASE DISREGARD ANY AUTOMATED REMINDER EMAILS YOU RECEIVE FROM OUR CLINIC, THESE MAY SHOW A DIFFERENT TIME THAN WHAT YOU ARE ACTUALLY SCHEDULED FOR. Credit Cards will be the only accepted form of payment.  

Please be prepared to be at our office for an extended period, especially if you are having films done or have multiple dogs.  We appreciate your patience.

CLINIC REGISTRATION CLOSES MONDAY MARCH 9TH

If you need you make any changes to the services being performed please do so no later than MARCH 23RD or we may not be able to accommodate your additional requests.
