Ultrasound Referral for Veterinarians

Please print the document that pertains to your patient and have it filled out for your owner to bring along with the patient to the referral appointment.  This gives us a better understanding of the reason for the ultrasound and gives a more detailed history for the consultation services.   When the results are received they will be submitted back to the referring hospital for review and treatment.
