Ultrasound Referral for Veterinarians

Referral Instructions

To refer a patient for ultrasound services, please complete and submit our referral form found below along with the patient’s medical records to info@sylvanvet.biz.

Once the referral has been received and reviewed, our team will contact the client directly to schedule the ultrasound appointment.

Our level of communication, case involvement, and reporting is tailored to the referral model selected on the referral form. Based on your preference, results may include a formal imaging report only, or may be accompanied by detailed diagnostic interpretation, case guidance, and direct collaboration.

We are committed to maintaining clear and timely communication while respecting the role of the referring veterinarian. Regardless of referral type, all findings and results will be shared appropriately, and you will be kept informed according to your selected level of involvement.

As a reminder we are now scheduling ultrasounds on Tuesday, Wednesday and Friday

Ultrasound Admission Instructions

Our ultrasonographer, Lauren, performs ultrasound referrals on Tuesdays, Wednesdays, and Fridays.

Admission Times:

  • Tuesday & Wednesday: 8:15–8:45 AM
  • Friday: 7:30–7:45 AM

 Digital Admission Form:

  • You will receive a digital admission form via email prior to your appointment.
  • Please complete this form before arrival on the morning of your pet’s ultrasound to help streamline the check-in process.

Fasting Instructions:

  • No food after 9:00 PM the night before the procedure.
  • If your pet requires sedatives, you may give a small amount of food to administer the medication, but please avoid a full meal.

 Sedation Information:

  • Sedatives are sent home with all current patients to help your pet relax during the imaging process and allow us to obtain higher-quality images.
  • If you are not a current patient and feel your pet would benefit from sedation, please speak with your primary veterinarian (pVMD) about prescribing medication prior to the ultrasound appointment.

Results:

  • Ultrasound results are typically received within 3 days.
  • Reports will be sent to your primary veterinarian for interpretation.
