Referral Instructions

To refer a patient for endoscopy services, please complete and submit our referral form found below along with the patient’s medical records to info@sylvanvet.biz.

Once the referral has been received and reviewed, our team will contact the client directly to schedule the appointment.

Our level of communication, case involvement, and reporting is tailored to the referral model selected on the referral form. Based on your preference, results may include a formal report only, or may be accompanied by detailed diagnostic interpretation, case guidance, and direct collaboration.

We are committed to maintaining clear and timely communication while respecting the role of the referring veterinarian. Regardless of referral type, all findings and results will be shared appropriately, and you will be kept informed according to your selected level of involvement.
